I just fixed mine and it was a missing ground strap on my Pertronix module.  It also had a bad capacitor on the ignition coil and the Pertronix wires were routed close to the spark plug wires.  My tach was going nuts most of the time but worked ok every once in a while.  Tach is perfectly smooth  now.
