Bob, you were right , incorrect installation over the fork. While my wife held the clutch pedal down, I used a small clevis pin. It greatly eases the installation of a cotter pin or a clevis pin by moving the fork closer to the opening.
Thanks to all for the advice. It is now correctly located and assembled.

Sounds like your rod end was installed over the fork and not in the fork. To let you know....you can re position the clutch rod with the power train installed.
I recently did this little job on my '64 Monza................ I used needle nose pliers to insert the cotter pin once the pin was in place. This was done by trial and error. With the cotter pin in place rotate the pin 180 degrees so the open end of the cotter pin faces front and bend the cotter pin ends up and down. (without pushing it back out)
It's not so bad to do.
