Hi Jerry,
YES! It is a known failure mode for Ultravans that might have vehicle
weights of over 5,000 lbs. A vast majority of these vehicles have
Powerglides and if the vehicle is allowed to downshift with the throttle
at the floor when climbing a hill the shock can break the pinion gear free.
