<VV> Broken bell housing & flex plate

The converter snout pilots in the end of the crank.  The pilot hole and the snout are worn.
The flex plate broke around the outside of the bolt circle in a jagged pattern but it did not come apart.

What do you mean the pilot hole is loose? It's a hole; holes don't move. 
There should be no reason to shim the pilot hole.
Describe the failure of the flex plate.
