Besides having a specific form, The title, ownership record.  AKA "pink 
slip" (not actually pink anymore) in California has (for the  last 25 years or 
so) a special tearoff portion at the top of the ownership  record for the 
selling party to fill out. It already has the information on  the car entered. 
It has a place for the selling party to enter all the  buyer and seller 
information as well as the selling price (or, at least,  the agreed upon 
number!) and the date. This portion of the form stays with  the seller, and the 
SELLER sends it into the DMV - or he should. The DMV must  now be notified 
within 5 days of a sale.  That submission constitutes the  release of liability 
for the seller.  I always make a copy before sending  it in.
