My guess is, it doesn't touch the belt unless the belt starts to whip against it. And the crank pulley is about, what, 6"? And this thing is closer to 1.5", I am guessing. So, not so extreme, especially since it doesn't turn most of the time.

So, if the crankshaft pulley is, what, 7 or 8 inches in diameter? And the
little idler is about, maybe 1 inch? So if the crankshaft were running at say
5,000 rpm, then the little idler would be singing along at 35 ~ 40,000 rpm!!!
That little guy must have some really good bearings!
