 It's an inertial damper used to absorb torsional oscillations of the crankshaft. The damping medium is the rubber that bonds the pulley to the hub. And yes, they do 'wear' out, usually by having the rubber ring start to slip, throwing the timing marks off, and ultimately, coming off the hub in a very ugly manner. And, sometimes, the seal land becomes too scored for the seal to work, as the other mode of failure. 
The term 'balancer' is actually a misnomer.

I know where it goes but am clueless about what it does. Why is it called a 
harmonic balancer?
And do they fail or where out?
