<VV> RE: Flywheel de-part-ure -- bolted, riveted, welded, solid ???

All these flywheels that have gone to outer space -- what were they??  
Stockish riveted, bolted, welded, solid steel, solid aluminum, 
lightened, early style or stepped (imagine most racers would use 
stepped) ??  Just wondering if one style was more susceptible to this 
failure mode than others -- I know, they all can do it ...

Same for clutches -- and, if known (I know it's hard to tell often 
times), which blew up first, clutch or flywheel?

ah, the gory details, but y'all can leave out the personal injury stuff 
if you want ...   ouch!
