I have three 5/8-11 setscrews that I had tapped for 3/8-18 installed in my
block. My PO pulled at least 3 studs and went back in with 1.5D long (.562
long) helicoils - too short, so they pulled out later. Probably smarter to
start with threaded rod. Setscrews are HARD.

repaired and then pulls through again? Is there an alternative type of
repair in this case, do you just use an over sized helicoil for round 2 or
