Bob,
This was the only 62 "Owner's Protection Plan" book I'd seen... it was in a 
batch of stuff I got at a swap meet in Ohio several years (10?) back... it was 
as if it was "carbon copied" from a protecto-plate, but this was just the 
impression, NOT the plate itself...
I have no idea where it might be, but we'll be digging out the Christmas 
decorations soon, so I'll check the closets when I'm in there.
