Two things which I did not see mentioned in this thread (I may have 
overlooked them).

The pilot bushing should NOT be ferrous - test with a magnet.  If it sticks 
to the magnet, don't use it.

The input shaft must be absolutely straight.

Attachments are scanned with anti-virus software.
