Agree with Al Birks. Year and model should be listed first. Also, you may want to consider a line for CORSA Chapter/Club Affiliation.

You may want to put the Class and Number sections after the Logo so all of the info is grouped together. I know someone will want to know where the car number is located. 

Also, maybe print the form in landscape instead of portrait so that it lays better on the dash.
