[FC] clutch symptom...cure?

I've adjusted the clutch cable about all the way out (shorter), and it's
actually pulling the lever or pin or whatever it's called out of the tranny
quite a ways, even BEFORE depressing the clutch pedal. We still get grinding
gears and a difficulty shifting into reverse, it's acting like the clutch isn't
getting engaged. This was a gradual problem, which rapidly got worse over the
last year. 2-3 years ago we had Mels' Vairmart replace the clutch (his guys
didn't tighten the nut on the adjuster, so it loosened and stopped engaging the
clutch by the time I drove it 60 miles home; I corrected that at the time).
So, do I need another clutch, a throwout bearing, what?
